Case Management Coordinator

NAMI New Orleans seeks a Case Management Coordinator with a masters degree in social work, sociology, psychology, or counseling for a full-time position in the Intensive Case Management program. Essential Functions

  • Management of case flow of clients receiving CPST/PSR services.
  • Completion of intakes and authorizations for new referrals and assignments to caseloads.
  • Tracking client Medicaid eligibility.
  • Provide community outreach to mental health centers, group homes, and hospitals and participate in discharge planning as needed to ensure continuity of care.
  • Provision of CPST services to clients.
  • Provide leadership to small team of case managers, assisting with service provision in employee absences.
  • Completion of monthly team report.
  • Manage small caseload and document service delivery daily.

Clinical Services Director

NAMI New Orleans seeks a driven and meticulous leader with an LCSW or LPC license for a full-time Clinical Services Director position. Essential Functions

  • Oversee and supervise the NAMI New Orleans Mental Healthcare Navigation Team (MHNT), the Uptown Drop-In Center, the NAMI New Orleans Community Awareness and Education Program, and the Hospital Navigator programs including paid staff as well as volunteers and interns.
  • Become proficient in Epitomax, an electronic health record (EHR), in order to provide in-depth records review and utilization oversight across the agency.
  • Act as a liaison between Epitomax staff and NAMI New Orleans staff that utilize the EHR. Conduct outreach and engage Epitomax staff to correct issues, make revisions, and/or update Epitomax as needed.
  • Perform intakes, assessments, and authorization requests for the housing programs (Finally Home and PHILP). This may include other programs when needed.
  • Provide individual counseling services.
  • Document all billable services, assessments, and treatment plans in the electronic health record on the same day.
  • Conduct and attend staff meetings and supervisions when needed.

Licenses and Other Requirements

  • Must be a Licensed Clinical Social Worker or Licensed Professional Counselor
  • Must provide proof of licensure and completion of graduate education

Licensed Mental Health Professional

NAMI New Orleans seeks a full-time Licensed Mental Health Professional for its Womanspace Program. Womanspace is a transitional shelter for women and transgender individuals of all genders who are chronically homeless and live with chronic mental illness or co-occurring disorders. Essential Functions

  • Oversee the Safe Haven case load, and provide crisis intervention services, as necessary
  • Review the file of each participant for internal consistency and completeness
  • Ensure the timely and accurate completion of each participant’s re-authorization with each Healthy Louisiana Managed Care Organization (MCO)
  • Engage in dispute resolution with Safe Haven participants, communicate directly with participants and case managers during those occasions, and provide appropriate clinical interventions, when necessary
  • Document participant non-compliance or suspected fraud
  • Mentor case management employees while they are mastering new responsibilities. Provide leadership and guidance as needed
  • Cover the caseloads of other case managers during times of vacation, leave of absence, or staff vacancies
  • Audit new client files, and do at least semi-annual quality assurance, as assigned
  • Terminate participants for non-compliance with program rules and policies, in coordination with the Transitional Housing Program Director
  • Review/audit clinical staff notes for completeness, and intervention effectiveness
  • Attend all required training and safety meetings; use, maintain and replace assigned personal protective equipment; report all injuries and illnesses to supervisor immediately; complete and submit incident/accident reports as required; report hazardous conditions or unsafe work practices to supervisor; and follow all safety and health guidelines, practices, policies and procedures. Monitor and enforce the agency’s Safety Program, and actively support and participate in Emergency Preparedness tasks.
  • Must be on call, on a rotating basis with Transitional Housing Program Director, Overnight and Evening Director, and case management staff.
  • Perform any/other related duties as assigned.

Required Experience and Education

  • Masters degree in social work, sociology, psychology, or counseling
  • Four years related work experience, including a minimum of 12 months experience working with individuals living with serious mental illness and individuals who are transgender

Licenses and Other Requirements

  • Must be a Licensed Clinical Social Worker or Licensed Professional Counselor
  • Must provide proof of licensure and completion of graduate education
  • Must have a valid driver’s license, reliable transportation with insurance, and a clean driving record

Housing Case Manager

Case Manager

  • Coordinate all safe haven residents into permanent supportive housing within 60 days of entry
  • Provide group counseling
  • Participate in an on-call emergency phone rotation
  • Work at least one weekend day shift per month

Internships


NAMI New Orleans accepts 1-2 interns a semester in the areas of social services, public health, communications, et. al. Opportunities best suited for undergraduate students. At this time, the agency cannot provide internships for social work, counseling, or any other degree that requires specialized oversight or direct interaction with clients.
